Integrated Care Partnerships: Update

The implementation of proposals developed by ICPs, and agreed by Local Commissioning Groups, is now underway across all areas. A number of service changes are now live including, a social prescribing pilot in the West, an integrated respiratory service in Belfast, a nursing home in reach service in the North, a social care response pilot in the South East, and an expanded acute care at home service in the South.

  • Award success for ICP pharmacy initiative – An ICP pilot service, led by Craigavon/Banbridge ICP Chair and Pharmacist lead Raymond Anderson, to help patients to manage their long term conditions at a local pharmacy is named ‘GP Partnership of the year’ at the 2015 C+D (Chemist and Druggist) awards in London.
  • Northern respiratory initiatives underway – Dr Carol Dalzell, Chair and GP lead of the Mid Ulster ICP, outlines the range of improvements to existing services and more joined up working between service providers for COPD patients in the Northern ICP area.
  • Western ICPs support new end of life care service – Western ICPs play a key role in bringing together the healthcare professionals, voluntary and community representatives and service users and carers for the innovative ‘Compassionate Communities’ project.
